Step 1 — Create a single “Evidence Vault”

Pick ONE home (OneDrive/Google Drive). Create folders: 00_Index, 01_CPD, 02_QI, 03_SignificantEvents, 04_PatientFeedback, 05_ColleagueFeedback, 06_ComplimentsComplaints, 07_TeachingLeadership, 08_CertsIDs, 09_ApplicationsPack, 10_AppraisalExports.

Step 2 — Build a Master Index (spreadsheet)

Create columns: Date, Domain (CPD/QI/etc), Role/Scope (GP/SHO/Locum), Title, 1-line Impact, Evidence Link (file), Reflection Link (note/PDF), Tags (ST3/Trust Grade/ARCP), Last Used (where), Notes.

Step 3 — Adopt strict naming rules (this is the cheat code)

Name every file like: YYYY-MM-DD__DOMAIN__TOPIC__SETTING__OUTCOME.pdf (example: 2026-01-12__QI__SepsisBundle__ED__+12%Compliance.pdf). Searching becomes instant. Shortlisting packs become “drag and drop.”

Step 4 — Capture evidence as “micro-packets”

Each activity should produce: (1) a proof artifact (certificate, meeting minutes, feedback form, screenshot, summary table) and (2) a 6–10 line reflection focused on learning + next action. Don’t write novels; write decisions.

Step 5 — Weekly maintenance (15 minutes)

Every week: add 1–2 items to the index, write 1 reflection, and tag anything that maps to upcoming applications. Your future self will thank you.

Step 6 — Create an “Application Pack” export folder

Keep ready-to-deploy versions: CV (NHS + Industry), Supporting Statement shell, 3 STAR examples, QI one-pager, Teaching one-pager, Leadership one-pager, and a PDF bundle of “top 10 evidence.”

The 5 portfolio mistakes that silently kill shortlisting scores
