If a child or young person is suspected of having learning disabilities and complex needs, consider referring them to education services for an assessment of their special educational needs (SEN) and disability (SEND) services, including the SEND Local Offer and SEND Information, Advice and Support services, after obtaining consent from the child and their family .

For broader support, evaluate whether there may be underlying health conditions requiring referral to health services, and involve them accordingly .

If social care support is needed, a referral for a social care assessment should include a detailed description of the reasons for referral, including emerging health and social care needs, and any barriers to accessing healthcare services, such as low income .

Discuss the potential outcomes of the referral, including an assessment of need, with the child and their family before proceeding .
