In which cases of upper GI bleeding should I consider urgent endoscopy for my patients?

Urgent endoscopy should be considered immediately after resuscitation in patients with severe acute upper gastrointestinal bleeding who are unstable. This includes those with signs of hemodynamic instability such as hypotension or tachycardia indicating severe bleeding.

For all other patients with upper gastrointestinal bleeding, endoscopy should be offered within 24 hours of admission. This timing allows for appropriate risk assessment and stabilization before the procedure.
