Refer patients to mental health services for further evaluation of suicide risk if they are feeling hopeless or are currently thinking about or planning to harm themselves or attempt suicide, especially if there are serious concerns or previous suicide attempts, as indicated by a high or sustained level of distress, or if they have a history of self-harm or suicide attempts .
