When should I consider referring a patient for cardiac rehabilitation after an MI?

Consider referring a patient for cardiac rehabilitation as soon as possible after a myocardial infarction (MI), ideally before hospital discharge. The cardiac rehabilitation programme should start within 10 days of discharge from hospital to ensure early initiation of recovery and secondary prevention.

All patients who have had an MI, regardless of age, should be offered a cardiac rehabilitation programme that includes an exercise component. This applies even if they have reduced left ventricular ejection fraction, provided they are clinically stable.

Referral should be part of a structured care system with clear responsibility for arranging cardiac rehabilitation. The programme should be tailored to the patient’s clinical needs and personal circumstances, including cultural sensitivity and accessibility.
