An 86-year-old man presents to the emergency department after being found on the floor by his neighbour. He is unable to recall what happened. He lives alone, has atrial fibrillation on Edoxaban, and has a CFS of 5. Examination reveals confusion (4AT = 6), bilateral bruising on arms, and a laceration on his forehead. CT head shows no intracranial pathology. He appears unkempt and underweight (BMI 17.2). The neighbour mentions she rarely sees anyone visiting him. What additional concern should be raised?
