A 70-year-old man with hypertension and osteoarthritis presents with vomiting and diarrhoea for 3 days. He has been taking Ramipril and Ibuprofen regularly. His creatinine has risen from 100 to 220 micromol per litre and urine output is reduced. He is haemodynamically stable. What is the most appropriate initial management?
