A 32-year-old woman with known asthma presents with severe breathlessness. She is speaking in single words, respiratory rate is 30 breaths per minute, pulse 124 beats per minute and peak expiratory flow is 35% of predicted. She has already received high-dose inhaled Salbutamol at home. On arrival she is given high-flow oxygen and nebulised Salbutamol and Ipratropium via oxygen-driven nebuliser.
