A 37-year-old woman presents with 6 months of intermittent crampy abdominal pain relieved by defecation, associated with bloating and alternating constipation and diarrhoea. She has no weight loss, rectal bleeding, nocturnal symptoms or family history of colorectal cancer. Physical examination, basic labs and CRP are normal. What is the most likely diagnosis?
