A 23-year-old man presents to the emergency department with severe breathlessness and wheeze. He has a history of asthma but has not used preventer therapy for years. On arrival he is speaking in single words, respiratory rate is 32 breaths per minute, heart rate 124 beats per minute and oxygen saturation 90% on room air.
