A 50-year-old man presents with chest pain and dyspnoea. ECG shows inferior ST elevation and he is reperfused with primary PCI. Two days later he develops pleuritic chest pain, low-grade fever and a pericardial friction rub. Troponin remains elevated but ECG now shows diffuse ST elevation with PR depression.
