A 35-year-old man with a history of intravenous drug use presents to the Emergency Department with a 2-week history of fever, night sweats, malaise, and generalised aches. He has lost some weight. On examination, his temperature is 38.6°C, HR 110 bpm. Cardiac auscultation reveals a new pansystolic murmur heard loudest at the apex. Small, non-tender erythematous macules are noted on his palms (Janeway lesions). What is the most likely diagnosis?
